Heart Health Assessment

The Heart Health Assessment was developed Healthy Heart Society as a teaching tool to help you gain a better understanding of your risk factors for developing cardiovascular disease, diabetes and other lifestyle related chronic conditions.

The profile is intended to be used in conjunction with the hearts@work program, and in consultation with a health professional. It is for screening and educational purposes only and is in no way meant to replace a physician's advise, diagnosis and/or treatment. 

The summary results you receive are NOT a medical diagnosis nor are they intended to be a substitute for medical care. We encourage all persons with risk factors in the RED zone to discuss the results of this profile with their family physician.
