Heart Health Assessment

The Heart Health Assessment (HHA) was developed by Healthy Heart Society as a tool to help you gain a better understanding of your risk factors for developing cardiovascular disease (CVD) and to provide you with information on how you can lower your risk.  

The HHA is intended to be used in conjunction with the hearts@work program, and in consultation with a health professional. It is for screening and educational purposes only and is in no way meant to replace a physician's advise, diagnosis and/or treatment. 

The summary results you receive are NOT a medical diagnosis nor are they intended to be a substitute for medical care. We encourage all persons identified as high risk to discuss the results of this assessment with their family physician.

Please Note:  If this is your first visit to the site you will be prompted to change your password.  Please choose a password that you will remember for future visits.

You will also be required to read and accept a user agreement and update your personal profile.
